HerzMobil Tirol is a specialised care programme for patients with heart failure. This aims to ensure the best possible therapy to stabilise the disease and at the same time improve the quality of life of those affected in the long term.

Optimisation of drug treatment: By gradually adjusting the medication in the first three months after the start of the program, the aim is to achieve optimum treatment success.
Promotion of health literacy: Patients and relatives receive comprehensive training to ensure safe handling of the disease and the prescribed medication.
Improving the quality of life: A holistic care network and the integration of modern telemedicine ensure close support that offers patients and their relatives greater safety and quality of life.

How HerzMobil Tirol works:

1. Support team as an interdisciplinary network

An interdisciplinary team consisting of hospital doctors, specialised nursing staff, general practitioners and other specialists provides patients with comprehensive care – from inpatient treatment to discharge and care at home.

2. Telemedical support

Using information technology such as a special smartphone app linked to scales, blood pressure and pulse monitors, patients regularly transmit their health data. This data is monitored by specialised nursing staff and doctors so that they can react quickly if necessary.

3. Training and consulting

Training patients and their relatives is a central component of HerzMobil Tirol. This takes place both in hospital and through home visits to ensure comprehensive information and the correct handling of medication.

4. Medication management

The medication is individually adjusted and gradually optimised. The aim is to continuously adjust the dosage in order to achieve maximum treatment success.

5. Exercise and lifestyle

Another important pillar of the programme is the promotion of exercise and a healthy lifestyle, which is discussed in detail during the training sessions and tailored to individual needs.

Contact and participation

Admission to the programme takes place through a referral in the hospital, either by the attending ward physician, the supervising nurse or an internist in private practice. Once the inclusion criteria have been met, HerzMobil Nursing contacts the patients to coordinate their inclusion in the care programme.
